Chronic Conditions

We are initially targeting six categories of chronic conditions that are driving up to 70% of the costs.

Within these categories we address 15 chronic conditions and there is a dominant commonality across the board.

The universal algorithm includes prevention, early intervention and management ... and the shortfalls in implementing the presccribed solutions (not just the meds) repeat themselves over and over.
