The primary focus is on the lifestyle side of the care plan where much more success is both possible and necessary.

We are working to describe the diabetes navigation plan within the LHIN ... what is the care plan(s) ... which stakeholder is responsible fopr each satep of the care plan ... how do we get more individuals completing the algorithm set out ?
