Self-Management
Our short term goals are to connect more people with the provincially-funded Stanford self-management program, to bring this program onsite in the workplace and to provide more effective follow-up support once the workshops have finished.
More people have to complete the education component before they can fully benefit from the Stanford component ... and after Stanford these "graduates" are supposed to be more ready to embark on behaviour change plans.
We are also considering how the Brief Action Plan can be implemented by practioners along side referring people to Stanford and other programs.
